Scans today revealed Segeyaro will need surgery on the syndesmosis rupture and he will be sidelined for a minimum of 14 weeks.
The Cowboys, who have the bye this weekend, have 11 matches left until the Telstra Premiership finals series begins.
The 22-year-old will see a specialist in Brisbane later in the week.
Segeyaro had played all but two matches so far this season for North Queensland to take his career total to 30, and won the starting hooker job for the Cowboys for the first time this year against the Titans.
